Placement and Transition

Those at risk for truancy, academic failure, and/or suspension, expulsion, or who have dropped out.
Those with emotional and/or behavioral problems and/or at risk of participation in juvenile justice.
Those who are returning from psychiatric hospitalization and/or a juvenile justice facility.
Those whose learning styles are better served in an alternative setting
Those who meet CANS (Child and Adolescent Needs and Strengths) and CALOCUS (Child and Adolescent Level of Care Utilization System) criteria with a level of need between 12 and 15.
A case conference will occur with the referral source and therapeutic team to discuss admission to the program. The staff will work with all relevant parties to develop a transition plan for returning to home school, which will consider the student’s ability to comply with a change in placement.

Referral Submission - Start by submitting the referral form online.
We Connect - A Thrive team member will call to confirm and/or gather any additional information.
Scheduling of Assessment - A Clinical Team member will review the referral and arrange a therapeutic assessment date for potential admission with the parent/guardian and child.
Assessment Result - Our clinical team group will analyze the assessment results, share the results with families, develop a tailored plan that fits the individual child's needs, and create an ongoing comprehensive plan.
Start and Orientation Date Scheduled - We will identify the start date and sign the placement and commitment agreement with parents/guardians.
